Endoscopic Ultrasound-Guided Fine Needle Aspiration (EUS-FNA)

Endoscopic ultrasound-guided fine needle aspiration (EUS-FNA) is a minimally invasive test that uses an endoscope with ultrasound imaging to obtain tissue or fluid samples for testing. What is EUS-FNA?

EUS-FNA combines multiple procedures into one. It uses a small tube with an ultrasound attached to provide detailed images. These images help doctors guide small tools through the tube to take a fluid or tissue sample (biopsy). 

The sample is then sent to a lab for testing. This helps doctors determine if the area of concern is cancerous or non-cancerous (benign), or the type of cancer if it is cancerous (malignant). This helps in early diagnosis of the cancer.

What to expect with EUS-FNA

EUS-FNA is an outpatient procedure, so you can go home the same day. It is typically done under deep sedation or general anesthesia. The procedure itself takes only 30-60 minutes, but you should prepare to be at the hospital for about three to four hours.

Frequently asked questions about EUS-FNA

How do I prepare for EUS-FNA?

Your doctor will give you details on how to prepare for your procedure. Let them know if you are taking any prescription medications. They will inform you which to keep taking and which you should discontinue.

For an upper endoscopy, you will typically need to stop eating eight hours before your procedure. In the case of a lower endoscopy, you may need to take laxatives to clear your bowel beforehand.

Arrange for a friend or family member to drive you home after your procedure. You may still be sluggish for a while afterwards.

What happens during EUS-FNA?

During an EUS-FNA, the endoscope is routed in one of two ways. For an upper endoscopic ultrasound, it goes through the mouth into the esophagus, then the stomach and into the small intestine. For a lower EUS, it goes through the anus and rectum to the large intestine. 

Your doctor uses the tiny ultrasound on the end to take images and help guide its direction. The ultrasound uses high-frequency sound waves to create images inside the body.

When the endoscope reaches the area of concern, the doctor uses a tiny needle attached to a syringe to collect a sample of fluid or tissue for testing. More than one FNA may be done to collect samples from different areas. The sample is then sent to a pathology lab for testing. 

Are there risks or complications with EUS-FNA?

The most common risks of EUS-FNA include:

  • Bruising or minor bleeding
  • Sore throat
  • Swelling

Rarely, people may experience:

  • Infection
  • Injury to the organs close to the area that was biopsied
  • Pancreatitis
